Valley Hospital is seeking a dynamic and dedicated Admission and Discharge Officer to join our team. As an essential member of our healthcare community, you will play a pivotal role in ensuring seamless transitions for our patients, from admission to discharge.

Key Responsibilities:

  • Manage patient admissions, ensuring accurate and timely processing of paperwork and documentation.
  • Collaborate with healthcare providers and insurers to facilitate smooth admission procedures and ensure compliance with regulatory requirements.
  • Conduct comprehensive assessments to determine discharge needs and develop discharge plans tailored to individual patient requirements.
  • Coordinate with interdisciplinary teams to implement discharge plans and ensure continuity of care for patients transitioning from hospital to home or other care settings.
  • Provide support and guidance to patients and their families throughout the admission and discharge processes, addressing concerns and facilitating smooth transitions.
  • Maintain accurate records of admissions, discharges, and related documentation in accordance with hospital policies and regulatory guidelines.
